About this volume
Volume IV converts the diagnostic and biologic information from the first three volumes into an oncologically sound first operation. It distinguishes parotidectomy extent, facial-nerve preservation or sacrifice, deep-lobe and parapharyngeal exposure, other salivary sites, cN0 versus cN+ neck management, reconstruction, complications, and specimen communication. Residents learn to plan around credible extensions of disease, not the most reassuring biopsy alone; use focused frozen-section questions; preserve an intact, separable nerve; prepare reanimation when sacrifice is plausible; and map margins and nodal basins for later treatment decisions. Integrated cases make the same procedure name look appropriately different for low-grade superficial disease, high-grade nodal disease, nerve involvement, or deep-lobe tumors. It is the operative phase of the series, before Volume V addresses adjuvant treatment, recurrence, and surveillance.
After completing this volume, the resident should be able to
- Plan parotid and other salivary operations around credible disease extent rather than procedure labels.
- Reason through facial-nerve preservation or sacrifice, reanimation readiness, and cN0 or cN+ neck management.
- Use focused frozen-section questions and specimen mapping to support margin and adjuvant decisions.
